I'm sorry to be obtuse, but does this mean that it cannot be ordered
from 1-800-537-5950, which is where I ordered PAF?
And are the "local distribution centers" the local Family History
Branch Libraries?

Yes that is the one. That is the toll free (North America) number for the
distribution center in SLC. There are many more distribution centers around
the world.

No they are not.
Not unless a FHC choose to do so, (and they are not supposed to) will they
have the program for sale.

It might help if you did not put the hospital in the birth(I guess it is a birth)
field.
The PAF 3.0 book on page 182 says it is ok to put in place field but is really
preferable to put this in the notes. I would put the geographic place in the
place field and the hospital in the notes like:
